Filling out forms can be time consuming. If it is your first visit to our office, please click the link below so that your history will be ready to review during your appointment. If you are an existing patient we will generate a "passcode" that you can enter to review all of the information that we have on file for you.
We respect our legal obligation to keep health information that identifies you private. We are obligated by law to give you notice of our privacy policies. This Notice describes how we protect your health information and what rights you have regarding it. Please take a moment to review it and ask us if you have any questions or concerns.
We want your eye exam to be as pleasant as possible. We would also like you to be well informed of our procedures before you visit so that you may plan accordingly. Our doctors prefer the Optomap Retinal Exam for every patient, every year. This is NOT covered by insurance and is currently $39 in addition to your copays. (It IS INCLUDED if you are not using insurance.) If you prefer NOT to have the optomap, we will ask you to dilate your eyes. For more information see the "Alternative to Dilation" link to the right.

We would like you to be as informed as possible before your visit so that we may answer any questions that you may have before your exam. This page discusses our billing policies and information regarding how we must file your insurance. There is also a section for contact lens wearers, if applicable to you.
